What is home Health Care?
Home health care is the involvement of trained professionals in the provision of health care and health-related
services to a patient in their own homes.
As a result, many people can lead healthier, happier and more independent lives. Patient and family education is an
important part of home health care.
The involvement of the patient and family members in the treatment plan often improves the effectiveness of the
plan. All services are provided under the direction of the patient's doctor. Services are provided by trained
professionals such as:

How Can I Get Care at Home?
To get home health care service,
- Patient must be under the care of a physician.
- Patient's physician must decide that patient needs medical care in their home.
- Patient must require skilled medical care.
- The service needed must be reasonable and necessary.
- Patient must be homebound, meaning leaving home is a major task.

What is Skilled Nursing
Services?
Registered Nurses provide professional health care to patients at home. Nurses deliver a full range of
health care to the patient as directed by their physician.
Examples of services offered by these professionals are:
-- Physical assessment, evaluation and observation
-- Wound care
-- Administration of medication, including injections
-- Parenteral and enternal feedings
-- IV Chemotherapy
-- IV Antibiotics
-- Oxygen provision and instruction
-- Catheterization/catheter management
-- Monitoring of cardiovascular and cardiopulmonary status
-- Home environment safety assessment
-- Instruction on and information on diabetes, hypertension, heart
attacks, and ostomy care
-- Family instruction on patient care
-- Post-partum visits including evaluation and instruction
-- Laboratory specimens - (drawing blood, collecting urine specimens,
cultures, and returning them to the lab)
-- Nutritional instruction
-- Refer to community resources as appropriate

What are Home Health Criteria for
Service?
The patient has a functional limitation that makes them homebound. (ex: no weight bearing, post-surgical
limitations, open wound)
The patient must have the need for a skilled service. Skilled nursing (IV fluids, open wound care) and/or
physical, occupational, or speech therapy.

What if I am in a Medicare Advantage
Plan?
If you belong to a Medicare Advantage plan, your choice of home health
agencies may be limited to agencies that contract with or agree to accept
the Medicare Advantage plan. Call your plan if you have questions about the plan's home health care rules. If you
get services from a doctor
or a home health care agency that doesn't work with the plan, neither the plan nor Medicare will pay the
bill.

What is Medicare?
Medicare is a health insurance program for people 65 or older or
people under 65 with certain disabilities or end-stage renal disease.
Medicare does not cover all expenses and is not designed to pay for long-term custodial care. so it is important to
understand the program.
For patients meeting requirements, Medicare helps cover the costs for hospital stays, skilled nursing home stays up
to 100 days and hospice care.

What is
Medicaid?
The Medicaid program provides medical benefits to low-income people
who have no medical insurance or inadequate medical insurance.
The federal government establishes general guidelines for the program, but each state establishes the program's
requirements including eligibility.

What is
hospice?
Hospice is a special way of caring for people who are terminally ill,
and for their family. This care includes physical care and counseling. Hospice care is given by a public agency or
private company approved by Medicare. It is for all age groups, including children, adults, and the elderly during
their final stages of life.

Who is eligible for Medicare hospice
benefits?
Hospice care is covered under Medicare
Part A (hospital insurance). You are eligible for Medicare hospice benefits when:
You are eligible for Medicare Part A (hospital insurance)
Your doctor and the hospice medical director certify that you are terminally ill and probably have less than six
months to live
You sign a statement choosing hospice care instead of routine Medicare covered benefits for your terminal
illness
You receive care from a Medicare-approved hospice program.
Medicare will still pay for covered benefits for any health problems that are not related to the terminal
illness.

How long can I get hospice care?
You can get hospice care as long as your doctor certifies that you are terminally ill and
probably have less than six months to live.
Even if you live longer than six months, you can get hospice care as long as your doctor recertifies that you are
terminally ill.
Hospice care is given in periods of care. As a hospice patient, you can get hospice care for
two 90-day periods followed by an unlimited number
of 60-day periods. At the start of each period of care, your doctor must certify that you are terminally ill in
order for you to continue getting hospice care.
A period of care starts the day you begin to get hospice care. It ends when your 90 or 60-day period is up. If
-your doctor recertifies that you are terminally ill, your care continues through another period of
care.

what is your hours of
operation?
Our office hours are Monday through Friday from 9:00 AM to 5:00 PM except on New Years day,
Memorial day, July 4th, Labor day, Thanksgiving and Christmas day.
After Hours Coverage: We provide 24 hour on-call service, 7 days per week to ensure that you
receive adequate medical care. However, in life threatening situations go to the hospital emergency room or call
911.
Weather Conditions: During the snow, ice and flood season we will make every effort to
continue home care visits. But the safety of our staff must be considered. When roads are too dangerous to travel,
our staff will if possible, contact you by phone to let you know that they are unable to make your visit that
day.
